If you want to publish your form somewhere else than a post, page or sidebar widget, you can copy the code in any theme file, using the Function Call option.
This option may be used to publish your form anywhere on your websites, such as your header or footer. Note: Take into consideration that some PHP knowledge is needed to do this safely.
