The Widget method of publishing is suitable if you want to publish the form in a sidebar widget. It is located in the Appearance → Widgets section of your collapse menu. 
Here you can drag and drop the CaptainForm widget in the sidebar of your website. Once you did that, simply select the form you want to publish, from the dropdown menu.
